New Client Process
Life Resources offers a peaceful, safe, and confidential place to begin your healing journey. It takes courage to seek help and we are here to support you every step of the process.
To become a new client, please complete the appropriate intake form below and our Client Care Team will contact you within two-three business days. Here at Life Resources, we pair clients with a therapist that best suits the specific needs of the client, such as location, insurance, etc. Our therapists are all licensed and specialized in specific areas that aid our clients in their healing process.

All new clients are required to pay a non-refundable $30 registration fee.

- Adult Counseling Intake Form (Clients that are 18 years old or older.)*
- Child/Adolescent Counseling Intake Form
*If participating in marriage counseling, both spouses are required to complete an Intake Form individually.
